Go to jcharper.net/family and create an account using your name. I will activate it. When you log in, look on the left in the Contents Menu for Family History. There you will find a Family menu on the right. To see (and test, if you wish) the form, click "Introduce Your Family". To see all introductions, choose "All Introductions".

I will have to delete it when the site becomes active since I promised that only family members would have access.

there is a "code" tag in your HTML source code, that is responsible for the srewed layout.

Did you by any chance copy the plugin string from the documentation and paste it into your website?

In the documentation on our website, the example strings are wrapped in HTML code tags, in order to set them appart from the rest of the text. If you copy the string from the documenation, sadly sometime the code-Tag is copied, too, and if you paste the string into your article, this will insert the code-Tag, too.

How to fix it?

Open the article where you have entered the plugin string. Toggle the editor, so that you can see the HTML Source code. Check, if there is an <code> and a matching <\code> tag and remove them (or maybe there are even more).
