Good analysis Aicha, yes I am using XAMPP with PHP 5.4.27. I apologize for not mentioning this before.

Soon I will be uploading the site to a test website and I will try your code without the changes and report back the results.

I finally was able to reproduce the notice and the problem with the select on my test system.
At the moment I'm working on fixing these problems, as well as a bug in the Javascript that is used to create options for selects, checkbox groups and radios.
I'll let you now, when I release the update with the bug fixes.

I just released Visforms 3.2.2 (it's available in the download section). I refactored the code, so that the notice should no longer appear. I decided on the fix with array('class' =>').

The help of users who are willing to work with and by that test beta versions is invaluable.
