currupt style of second date fields

Hi,
I've installed VisForms 3.4.0 on Joomla 3.4.0 and I have a problem:
If I put one date field in the form everything is ok but if I put a second date field after the first one, the second one shows a "button" instead of calendar icon and the icon goes in another place.
This is what I obtain with one date field:



and this is the result with two date fields:



If I move the second field two or more places below it shows correctly.



I've tried with different Joomla installations and different templates: always the same result.

Any help?

Need to look at the form itself not some screenshots.
