Label name not showing correctly

Label name not showing correctly was created by akshehu
Hi..
i tried to fix but could not find where to adjust it.
1. In the showing the data "As a list". The label and the data field is together without space or ":". 
2. Instead of number , it is showing "Index" as count for the data submitted 
 

How to correct this or is it a bug?
