select id as value, F1 as label from aspt_visforms_1

the as value and as label must be there!
value and label which are returned by the sql statement are use to create the options

Sorry I did not understand Indeed my SQl request was not good. Now it works: SELECT id as value, F1 as label FROM `aspt_visforms_1` limit 20
