Only Form Displays with Visform menu item

I hope you can assist as I love your extension and bought extra plugins for this form so I don't want to use another extension. I need help my site where all the site formatting and menu items disappear when I set Visform as a menu item or module. You can only see the form and nothing else. Would you be able to take a look at this?

sometimes while developing Visforms I see such screens, too. But at the moment I cannot remember exactly when this happens, because during development this usually is quite obvious. So I have to consider your problem a bit.

Could you please give me some more information
- Joomla! Version
- Visforms Version
- Did the form work in the past and just stopped for some unknown reason or is this the first time you display the form

This is a new joomla installation and I've only installed the extension now so this is the first time I'm using it on this site.

Joomla! 3.5.0 Stable
Visforms Version 3.6.11
First time use on this site

I have also installed the Visforms - Custom Mail Address plugin and enabled it but not yet configured

thanks for the additional information.

No the problem is not related to this forums post but it is good to see, that your tried to figure it yoruself and search the forum!

For some reason the document is not rendered correctly. The header (the html head tag) is completely empty. And only the form is rendered inside the body.
So all informations necessary to render the page (like css files, javascirpt...) are missing and that is, why everything looks so bad.

Not having configured the plugin does not cause the problem because this plugin is only invoked, after the form is sent.

We have to figure some potential sources for the problem.
Please test, if the page looks different, when you use the Joomla! "Default" template Protostar, at least for this menu item.
You could create a second very simple form (one text input one sumit button, no custom texts or special configuration anywhere) and try, if this form is rendered properly. Test this form with the Protostar template, too,

Okay so I created a new menu item with a basic test form with one field and enabled the protostar template and the main template and on both it displays correctly. But not on the custom form when I enable protostar:

Test form: aqratenew.cloudaccess.host/index.php/test

Custom Form: aqratenew.cloudaccess.host/index.php/bee-quote-request

So it seems the error is on my form. I am only guessing here but could it be because I have spaces in the text of the label on the select fields?
