Only Form Displays with Visform menu item

ok.
I think the next step would be to make a copy of the form with all fields (using the batch copy function of Visforms).
Create a menu item for the copied form.
And then reduce the copied form step by step.
After each change you have to check, if the form is displayed correctly afterwards.
I would start with removing all custom texts in the form configuration.
Try to reset the form settings to there defaults.
(No results or redirects in Result tab, no mails and no texts in Emails tab, no captchas in spamprotection tab, Default Layout in Advanced tab, all access levels set to Public)
Delete one field after an other.

Thanks so much for your advice. The process of elimination worked perfectly. In the end it was because I chose the ReCaptchas option under the Captchas settings in Visform without enabling the recapcha plugin that comes with Joomla.

Not sure why it would cause it to lose all the template formatting but very glad it is working.

I'm glad I could help.
For me it is always good to know what might possibly go wrong in the configuration of Visforms and I always try to improve the component. It is certainly a good idea, to check, if the plugin is enabled and do not show it in the list, if it is not. But it will take some time before I can do this.

In the meantime, people who suffer a similar problem hopefully find this post
